How to Apply


Eligibility

To be eligible for funding, an agency must:

  • Be a 501(c)(3) tax-exempt, not-for-profit agency.
  • Be an agency that meets The Blazer Foundation’s mission, which is to improve the lives of individuals with mental or physical disabilities living in Winnebago County and the adjoining counties of northern Illinois. The Foundation supports local initiatives in research and development, as well as programs designed to directly support those individuals with disabilities.

How to Apply

Only The Blazer Foundation application request form will be considered.

Applications should be submitted online by:

  1. Completing the online application found here, OR
  2. Downloading the Word Doc Application found here and submitting it to info@theblazerfoundation.org.

NOTE: Hard copy (mailed) applications will not be accepted.

Board Meeting Dates

The Board of Directors meets on a regular basis. The deadline to have your application reviewed at the next Board meeting is May 1, 2024.

Multiple Requests

The Blazer Foundation will consider multiple grant requests from an agency. Please submit one application for each grant consideration and submit all of your requests at the same time within the calendar year.

If the program requires multiple year funding, please address this in your application.

Follow-up

The Foundation requires written follow-up for all approved grants. The required information and schedule for follow-up will be provided.
